Authorization for Automated Deposit (PDF)
To establish a direct deposit of your pension payments into a designated bank account, complete this form online (login required). (After logging in, select Direct Deposit from the Benefit Recipient menu. You will be required to create a user name and password if this is your first time logging in or you have not logged in since May 3, 2012.)
As an alternative, you can download the direct deposit form here (PDF), or request a copy from the Fund Office. Return the paper form to the Fund Office.
Beneficiary Designation Form (PDF)
Active participants may use this form to name their preretirement death benefit beneficiary(ies).
You can also designate, view, and update your current beneficiary designations here (login required).
Beneficiary Designation Form for Benefit Recipients
Participants who are receiving benefit payments must submit changes to their beneficiaries in writing. You can request a paper copy of the appropriate form by sending an e-mail to firstname.lastname@example.org or by calling the Fund Office at 800-424-9608 ext. 277.
Non-participant Contact (electronic form)
Submit this electronic form if you are not currently participating in the plan and would like more information about the Fund, the National Pension Plan and IAM National 401(k) Plan.
Pension Application Request and Estimate Request Form (PDF)
You can apply for your pension online (login required).
You can also generate an estimate of your benefits online (login required).
You may also request a paper application for pension benefits or a written estimate by mailing us this Request Form or by contacting the Fund Office at 800-424-9608 and asking for the Pension Department. Review pension application request details here.
IAM National Pension Fund QDRO Model for Participants in Payment Status (PDF)
This model order may be used in drafting a QDRO for participants who are currently receiving pension benefit payments. It includes formulas for calculating the division of benefits among the parties.

Request for Ruling on Employment (PDF)
Submit this form to the Fund Office if you are considering returning to work after your retirement effective date. W4-P (Witholding Certificate for Pension or Annuity Payments) (PDF)
Complete this form online (login required) to indicate the correct amount of federal income tax to withhold from your pension benefit payments (login to your personal information required).
As an alternative, you can download the form here (PDF), or request a copy from the Fund Office. Return the paper form to the Fund Office.
Physician Statement of Incapacitated Benefit Recipient (PDF)
This form and accompanying statements to endorse checks or receive payments on a pensioner's behalf are to be used in cases of benefit recipients who are either physically or mentally incapacitated.
Forms for Surviving Spouse of Deceased Pensioner under Spouse Pension Option (PDF)
These forms are for surviving spouses who are entitled to benefits under a deceased Pensioner’s 50%, 75%, or 100% Spouse Pension. A death certificate is required to verify the death of a Pensioner.
